The Iberia Comprehensive Community Health Center, Inc. is soliciting responses to an RFP to select a verifiably interested and qualified firm or firms to provide completed design services based on drawings provided by David Bernard & Associates, 211 East Main Street, Broussard, LA.  The architect retained by the owner, David Bernard & Associates will work closely with the owner and D/BC to facilitate the understanding and operations of the clinic in order to complete the design of building and systems for a fully operational facility.
RFP responses will be accepted at David Bernard & Associates, 211 East Main Street, Broussard, LA. Attn: David Bernard until 1:00 p.m. Central Time, Tuesday January 17, 2012. An original and four copies of the RFP response must be submitted in a sealed envelope or box. Email or faxed responses are not acceptable. The Owner reserves the right to reject any and all proposals and is the sole determinant of timeliness and responsiveness. Submissions received after the deadline will NOT be accepted. All materials submitted in the proposal become the property of the Owner and will not be returned.

BID SECURITY equal to five percent (5%) of the Total Bid shall bein the form of a bid bond, certified check or cashier’s check as prescribed byLA RS 38:2218.A must be attached to and made a part of the bid. If a bid bondis provided it shall be on the provided form and only on the provided form. Bidbonds must be written by a qualified company licensed to do business inLouisiana. The successful bidder will berequired to furnish a Performance Bond and Payment Bond written by a companylicensed to do business in Louisiana in an amount equal to one hundred percent(100%) of the contract amount. Davis –Bacon Wage rules will apply to thisproject. For this project, LiquidatedDamages will apply in the amount of three hundred dollars ($300.00) per day foreach consecutive calendar day which the work is not completed, beginning withthe first day beyond the established completion date of 60 days. No bid may be withdrawn for aperiod of thirty (30) days after actual date of the opening thereof. The Iberia ComprehensiveCommunity Health Center Board reserves the right to reject any and all bids.
